MOQA (Measurements, Outcomes, and Quality Assessment) is a
county-driven, DHCS-supported effort to improve statewide
reporting on outcomes resulting from programs supported through
Mental Health Services Act (MHSA, Prop 63) funds. CIBHS supports
DHCS and California counties in designing measures, creating
tools, collecting data, and building reports in a consistent
fashion across the state. Currently, MOQA is focused on data
collection for Suicide Prevention (SP) and Stigma and
Discrimination Reduction (SDR) programs.
Pilot SP and SDR collection occurred from January 1, 2019 through
June 30, 2019. Data collected during the pilot should be
submitted to CIBHS by September 30, 2019. SP and SDR data
collection using tools refined during the pilot period continues
in Fiscal Year 2019-2020. Details on reviewing pilot data,
receiving pilot reports, collecting FY19-20 data, and receiving
technical assistance are available in the sections below.

Counties who submitted pilot data should have received a report
summarizing your data. Statewide and county-level narrative
reports will be shared in June.
MOQA SP and SDR FY19-20 data collection is very similar to pilot data collection, although we have made some small changes to the Participant Surveys based on feedback received during the pilot. Please follow the instructions below to collect FY19-20 data:
Participant Data
Download the PDF data collection forms or access the correct online survey for the data you wish to collect. Excel versions are provided to allow you to add or remove questions or to change formatting. Please do not change the wording of any questions, as that will invalidate your results. Threshold language surveys are available in the Threshold Language Resources section.
Contact CIBHS to submit quarterly data if you collect paper surveys. If you entered data into the online tool, you can skip this step.
You will receive a summary of the data you’ve submitted following each quarter and a comprehensive report after the end of the Fiscal Year.

The MOQA Program Data Collection Tool is designed to be completed once per year, at the end of the Fiscal Year. Download the template below to review the types of data MOQA collects. You can add data to the template at any time, and this section will be updated with instructions to submit data at the end of the Fiscal Year.

Q: What is the time frame for collecting MOQA SP and SDR
data?
A: MOQA is currently collecting SP and SDR data
for FY19-20. Participant and program data should be collected
from July 1, 2019 through June 30, 2020.
Q: What is the deadline for submitting FY19-20 data to
CIBHS?
A: CIBHS is working with DHCS on deadlines
for FY19-20 MOQA SP and SDR data, but we expect data to
be due to CIBHS by September 30, 2020. Please contact

TA to submit your data.
Q: I’m preparing my data to submit to CIBHS, but I need
some technical assistance to make sure I submit things correctly.
Who should I contact?
Q: I submitted my MOQA data through Survey Gizmo. How do
I access my data?
A: Contact Samantha Spangler at sspangler@cibhs.org to get an
extract of your data in Excel format.
Q: Can I use the MOQA surveys to collect data outside of
the pilot period?
A: Yes! You can use the MOQA data collection
tools to collect data at any time, but only data for activities
between January 1, 2019 and June 30, 2019 will be included in the
pilot MOQA SP and SDR reports. Data collected from July 1, 2019
through June 30, 2020 will be included in FY 2019-20
reports.
Q: Are counties required to collect MOQA data and submit
it to CIBHS for inclusion in statewide MOQA reports?
A: No, participation in MOQA is not required.
However, MOQA provides a unique opportunity for counties to shape
the way outcomes data is collected and reported in California,
and the more counties who participate, the better!
Q: Regulations only require us to submit aggregate
outcome and demographic data. Why does MOQA ask us to submit data
on individual participants?
A: Recording and submitting data on individual
participants with demographic data linked to outcome data allows
for more comprehensive data analysis and reporting. It allows us
to aggregate data more accurately across counties and programs
and facilitates additional analyses into questions of health
equity (i.e., are certain programs more effective for certain
demographic populations?). If you have questions about how to

Q: We serve a lot of participants in our SP and SDR
training programs each year. Is there a way to minimize the
amount of data we have to enter by hand?
A: Yes. CIBHS recommends that you share a link
to the online questionnaires to your program participants at the
end of the session so that participants with computers or smart
phones can enter their own data and county staff won’t need to
transcribe data from paper forms. Paper surveys can also be made
available for participants who prefer another language or cannot
access the electronic surveys, and CIBHS can help you merge the
data.
Q: Are MOQA surveys appropriate for all ages?
A: Yes! The MOQA surveys are appropriate for all
ages, and participants can select “Not Applicable” for any survey
item they feel does not apply to them. In addition, PEI
regulations do not require MHPs to report gender identity, sexual
orientation, and veteran status for individuals under age 12, so
those questions may be omitted or left blank for programs
targeting children.
Q: We use similar but different surveys for participants
in our SP and SDR programs. Can we still participate in
MOQA?
A: Yes! You can submit data collected via
different survey tools using the MOQA Program Data Reporting
Template. However, to be included in the statewide analysis of
MOQA participant data, you must use the exact wording and answer
choices in the MOQA survey tools. Data collected via other tools
will be reviewed and reported in aggregate only.
Q: Are you still accepting feedback on the MOQA data
collection tools?
A: Yes, we are always interested in feedback on
the MOQA data collection tools. However, to ensure data validity,
any changes to the surveys will be made at the end of the current
data collection period.
